I actually do not use my logbook because I prefer to have more space for comments and I like to use my logbook as a training and climbing journal. I use a standard lab notebook for my journal (with lined pages).
I organize it chronologically with alternating pages for training/climbing days and rest days. I find some reflection on what I did during my rest time can be informative to my overall training effort. (things like not getting enough sleep, or overly active in something else, being stressed etc)
In terms of organization, every right hand page in my book is a training day and every left facing page includes notes from the intervening rest days. On my training/climbing days I record the date, location, a summary of the weather/conditions, who I am with, and any other key detail from that day.
Then I have a section for describing my warm up.
Then a section for the primary workout. I model the workout sections partly on those provided in the logbook, but generally include space for qualitative notes related to each set, or route, etc. I also generally have room at the base of the page for summary notes or planning reminders for future sessions.
Another thing I like about the lined paper is that the lines for writing on are not as bold as they are in the logbook. I think a half tone line or some other way to make the lines and structure less bold would make the logbook easier to read.
